Fragmentation of a patient’s medical history under different medical record numbers has become a major concern of healthcare providers. Incomplete medical history has been responsible for critical errors related to patient care and patient safety. Healthcare providers, AHIMA, and JCAHO overwhelmingly agree that this increased risk to the patient is unacceptable.

Nearly all ADT systems fall short when providing data regarding how many duplicates exist in the Master Patient Index (MPI).  Many institutions have developed their own rudimentary routines to isolate the number at a high level. Medibase provides a more affordable and accurate solution that utilizes powerful probabilistic algorithms. By performing an in-depth analysis, Medibase is able to accurately determine the number and composition of all duplicates in the MPI along with estimates for achieving a permanent resolution of all MPI integrity issues.  

Using an extract of the facility's MPI or EMPI, Medibase analyzes the data to isolate all suspected duplicates currently in the system and delivers the following:

Analysis of the data elements that contain invalid or missing data values

List of suspected institutional or default data values

Criteria used to exclude MPI records from the analysis (e.g., trauma, institutional, and special tracking)

Counts and percentages of all suspected duplicates identified

Breakout by probabilistic score of all suspected duplicates

Detailed reports of every duplicate introduced into the system in the preceding 3 months broken out by month and responsible registrar

Percent of duplicates introduced during the 3 months reported compared to the total number of new patients added to the system during the same periods

Graphical depiction of where duplicates were introduced in the last 12 months broken out by patient type or service location

Printed and electronic summary/analysis of all Medibase discoveries along with possible approaches and strategies for cleaning the MPI

Medibase analyzes the full Master Patient Index using a sophisticated probabilistic scoring algorithm to determine whether there is sufficient evidence of duplication. Data normalization, phonetic mapping and transposition checking are just a few techniques employed to maximize the chances of finding duplicate patient and guarantor records. This process enables the cumulative evidence to be quantified into a single score used to determine which pairs are likely duplicates.

Complete details of duplicates introduced in the preceding 3 months provide insight as to how quickly the problem continues to grow. Medibase's side-by-side format of the suspected duplicates allows the customer to see exactly what data was entered versus what already existed in the MPI. It is valuable information for addressing causes that result in fragmentation of the patient's medical history.

From the customer, Medibase requires an extract of the MPI data in the prescribed Medibase format. Many facilities choose to provide information on where their patients initially enter the hospital so that Medibase can graph the entry points for suspected duplicates within the facility. This helps to pinpoint the specific intake trouble areas within the hospital where duplicates are most often created.
