MPI CLEANUP Request Information
With a focus on 'Best Practice', Medibase specializes in helping hospitals identify and resolve issues related to MPI Integrity (i.e., duplicate medical record, overlay of patient's medical record) which remains a serious obstacle to patient safety. In the last several years there has been increased awareness and scrutiny over inaccurate identification of returning patients and the need for clinicians to have access to each patient's full medical history. Although registration systems, hospital procedures and training have improved, additional measures are still required. Medibase works with clients to isolate the root cause for misidentification and has developed a suite of MPI Integrity Products to provide relief to hospitals, clinicians
and most importantly patients.
The impact of duplicate and overlay errors to the healthcare community has been extremely costly. However, once the situation is addressed and the integrity of the data improved, the facility will realize multiple benefits such as improved care, reduced returned mail, fewer denied/delayed claims and improved customer satisfaction.
The Medibase Philosophy

Experienced Medibase consultants can quickly evaluate the total patient intake
process and isolate areas that need improvement. Using extremely powerful
tools to assess the integrity of the hospital or enterprise MPI, precise recommendations
can be made to improve processes and rectify problems.
Comprehensive MPI Assessment
Medibase assists hospitals by performing a comprehensive assessment of the MPI.
Using an extract of the MPI, the assessment provides counts of all duplicates
currently in the system broken out by facility. It also identifies duplicate records that were added to the
system in the preceding 3 months. These are broken out by the responsible registrar and provide complete detail.
Graphs are also generated that clearly demonstrate the service areas within the hospital where duplicates are being created.

MPI Cleanup Staffing
Medibase provides experienced
staff to supplement internal HIM personnel in electronically
merging all duplicate medical record numbers to the correct
surviving number and consolidating the physical charts and
radiology film jackets to the single unique MRN for the
patient. Medibase resources work in concert
with experienced technical analysts to ensure that all
suspected identity errors are isolated, scrutinized and merged
when appropriate in all systems. Our team utilizes
the BASEmatch product to aid in
adjudicating suspected duplicate errors.
The cleanup team is conditioned to recognize unusual situations as well as common situations
involving babies, twins, trauma patients, family members and
institutional names. Medibase utilizes four levels in scrutinizing each
potential duplicate situation:
- Definitely the same person
- Probably the same person requiring verification
- Possibly the same
person requiring research
- Not the same person or there is no way to confirm that it is
the same person
Where the available client data does not
fully support that two identifiers are definitely the same
individual, web research tools that incorporate a variety
of public and private sources are used by the cleanup team
to prove or disprove that the pair represents the same
individual.
Each team is lead by experienced cleanup
managers who constantly interact with the client, provide
direction to the team and monitor decisions and
processes.
Medibase's technical resources not only
aid in analyzing data from the primary MPI, they can also
compare the data to data from any of the downstream systems to
isolate and report out of sync conditions. Each client
and cleanup project is unique. Medibase leverages a
strong technical background to provide alternative
solutions that reduce the cost of a cleanup and
to expedite the cleanup process.

with the cleanest possible Master Patient Index along with
tools and processes to keep it that way.
Project Management
Additionally, Medibase can provide experienced Project
Management to guide internal or external efforts to cleanup
MPI errors. This may be in the form of developing a
comprehensive RFP for a successful project or developing a
detailed Project Plan. Medibase can also provide oversight
management to continuously monitor contracted cleanup staff
and ensure that stringent HIM standards are maintained.
